Social Services at Burbank Healthcare & Rehabilitation Center provide support to residents and families throughout their stay. Our social services team works closely with residents, families, physicians, and the interdisciplinary care team to support care coordination, communication, and discharge planning.
Services are designed to help residents and families navigate care decisions and transitions while ensuring resident needs are addressed respectfully and appropriately.

What Social Services Provide
Care Coordination & Communication
Our social services team supports ongoing communication between residents, families, and the care team. This includes assisting with care planning discussions and ensuring families remain informed as resident needs change.
Discharge Planning Support
Social services staff assist with discharge planning, working with residents, families, and healthcare providers to help coordinate a safe and appropriate transition when discharge is recommended.
Resource & Appointment Coordination
When needed, social services may help coordinate:
- Medical appointments
- Community resources
- Support services related to ongoing care needs
Coordination is provided in collaboration with the care team and based on physician guidance.
Family Support
We recognize the importance of family involvement. Social services staff are available to support families with questions, concerns, and care-related planning throughout a resident’s stay.
