Social Services at Burbank Healthcare & Rehabilitation Center provide support to residents and families throughout their stay. Our social services team works closely with residents, families, physicians, and the interdisciplinary care team to support care coordination, communication, and discharge planning.
Services are designed to help residents and families navigate care decisions and transitions while ensuring resident needs are addressed respectfully and appropriately.
Social Services at Burbank Healthcare & Rehabilitation Center are part of a team-based approach focused on resident safety, dignity, and continuity of care. Support is individualized and adjusted as resident needs evolve.
Our social services team supports ongoing communication between residents, families, and the care team. This includes assisting with care planning discussions and ensuring families remain informed as resident needs change.
Social services staff assist with discharge planning, working with residents, families, and healthcare providers to help coordinate a safe and appropriate transition when discharge is recommended.
